Supplier Registration Forms

New suppliers may register by completing the Supplier's Application Form and the IRS Form W-9 (Do NOT fax application). M/WBE suppliers should also submit a copy of their City of Chicago certification letter or pending registration receipt.

Contractor Disclosure Form
Suppliers who submit a bid or proposal must submit the Contractor’s Disclosure Form. This form is usually included in the solicitation documents.

M/WBE Forms
(supplier use only)

Please note that these forms are not for obtaining M/WBE certification. If you wish to do business with CPS as a certified M/WBE supplier, you must obtain your certification from the City of Chicago. For information on becoming certified with the City, please read about M/WBE certification at the City's web site.

M/WBE forms must be submitted with bids and proposals to demonstrate compliance with the Revised Remedial Plan. The complete packet  consists of four (4) “Instruction” pages and eight (8) attached “Forms” titled 100, 101, 102, 103A, 103B, 104, 105 and 106. The forms found in this section can be filled out online. Here's how.

• Instructions - Revised Remedial Plan for Minority and Women Economic Participation
  Please read these instructions carefully before filling out the forms.
• Form 100 - Prime Bidder/Proposer Information - M/WBE Compliance Affidavit
  This form is required. If you fail to execute this form, your bid/proposal will be considered non-responsive and rejected. This form must be notarized.
• Form 101 - Direct/Indirect Participation of Subcontractors/Suppliers/Consultants
  M/WBE firms may participate in the performance of this contract either directly or indirectly as prime bidders/proposers; joint venture partners; subcontractors; and/or suppliers. If performing indirectly, approval by CPS is required.
• Form 102 - Joint Venture
  This form need not be submitted if all joint venturers are MBE's and/or WBEs. In such a case, however, a written joint venture agreement among the MBE and/or WBE venturers must be submitted. In all proposed joint ventures, each MBE and/or WBE venturer must submit a copy of their current Letter of Certification.
• Form 103A - Letter of Intent
  This form must be notarized.
• Form 103B - Non-Bid Professional Service Affidavit
  This form must be filled out by individual independent contractors or sole proprietors who are being retained by prime proposers to perform work. This form must be notarized.
• Form 104 Vendor Diversity Profile
  Provides CPS with a better understanding on the diversity make-up of the company.
• Form 105 - Request for Waiver
  This form is only required when seeking a waiver of the applicable M/WBE goals if despite good faith efforts, it is impossible or economically unreasonable to meet an MBE or WBE goal.
• Form 106 - Students Internship Agreement
  One way of demonstrating good faith effort, the Office of Business Diversity encourages companies to hire a student from the Education-to-Careers program and/or the Office of Specialized Services STEP Program

M/WBE Construction Forms
